The intent and purpose of this form are outlined in the adjoined cover letter. Along with these documents, the Chelsea High School Athletic Department is also happy to provide additional educational materials developed by U of M Neurosport (located at the bottom of this page) for further information on concussion and guidelines for safe return to athletic activity.
